Follow these instructions to receive a tax deduction for your wig / medical hair prosthesis.
Step 1: Get Pre-Approved
Before you purchase a wig, ask your insurance provider how much they are willing to reimburse you for a “medical hair prosthesis” (not a wig), and how long it will take to get reimbursed. If you’re insurance company requires a quote, you may download a sample quote below or contact us for a custom quote.
Step 2: File a Claim
In most cases, you can file a claim only after you purchase a medical wig. Simply download and send Claim Form #1500 to your insurance company. You will need the following items:
- Our Federal Tax ID: Coming soon
- Procedure Code from your insurance provider. Commonly used codes: Synthetic Wigs (A9282 or 99199); Human Wigs (L8499).
- Receipt paid in full.
- Rx with your diagnosis code from your doctor (it must be written for a “medical or cranial hair prosthesis”).
